Brief description of activity/activities (approx. 100 words): Summary of activity, aims and outcomes. Fit with NPME and YMH aims. If your project is a ‘continuation’ of a previously funded project then please show how this may be developed.

Explain how need was identified (approx. 50 words): Reference YMH needs analysis, school cluster level data, own analysis.

Describe how the activity will be delivered (approx. 400–500 words) Include target participants, who will deliver, where/when take place, length of project; plan for sustainability.

Please provide evidence of the quality of delivery of provision by the partners: Include an example of recent activity, evidence of good quality outcomes and relevant testimonial. Also include links to any CVs and websites, etc.
